Physician Assistant / Nurse Practitioner Visits
At Caraway Assisted Living, we prioritize residents’ health with regular on-site visits from Physician Assistants (PAs) and Nurse Practitioners (NPs). These healthcare professionals provide routine medical evaluations, monitor chronic conditions, adjust medications as needed, and address any emerging health concerns—all within the comfort of our community. Northside Pharmacy & Delivery Services
At Caraway Assisted Living, we partner with Northside Pharmacy to ensure safe and convenient medication management for our residents. Northside Pharmacy works with resident insurance, provides secure blister packaging for safety, checks for potential drug interactions, conducts semi-annual compliance audits, and collaborates with physicians to clarify prescriptions—helping to maintain residents’ health with accuracy and peace of mind.
Lab Services & Mobile X-Ray
At Caraway Assisted Living, we make diagnostic testing convenient and accessible with on-site lab services and mobile X-ray capabilities through coordination of services with local vendors and healthcare services. Our trusted healthcare partners provide routine lab work, communicate the results to the prescribing practitioner, and perform imaging as needed—all without the need for off-site appointments.
Delegating Nurse Oversight & Comprehensive Nursing Assessments
Our delegating nurse performs regular assessments to ensure each resident’s health and well-being is closely monitored. Using standardized healthcare practitioner forms and system checks, we track vital signs, evaluate medical conditions, and attempt to identify any trends or changes that may require intervention. Bi-monthly health reviews along with comprehensive assessments when a resident returns from the hospital or a rehabilitation center, allow us to proactively adjust care plans, collaborate with health care practitioners to provide timely medical support.
Physical & Occupational Therapy Services
Partnering with Total Rehab and Amedisys Home Care, Caraway offers physical and occupational therapy services that are thoughtfully designed to help residents regain strength, improve mobility, and maintain independence in a warm and supportive environment. Licensed therapists work closely with each resident to create personalized therapy plans that focus on their unique goals – whether it’s walking more confidently, recovering from surgery, or improving coordination for daily tasks – within their Caraway home.
Counseling & Behavioral Health Services
At Caraway, we take a close look at the behavioral and psychosocial health supports in place to ensure residents are thriving emotionally as well as physically. Practitioners from The Supportive Care Group, can perform standardized assessments help identify any shifts in mood, behavior, cognition, or engagement, allowing staff to respond proactively.
